Upcoming Enrollment Deadlines

New student referrals will not be accepted after these dates:

Currently due to Covid-19, the Home & Hospital program is practicing Distance Learning.

 

Home & Hospital is a program designed with temporary placement to help maintain a student’s academic standing during a time of illness or other temporary disability.  Students are entitled to a maximum of 5 hours per week of home instruction.  The teacher will be there at times agreed upon and at a minimum of up to 1 hour per day, per subject.  No sessions are allowed on holidays or weekends.  A responsible adult (18 years or older) must be present when the teacher is in the home.

The purpose of Home & Hospital is to support the student in maintaining a continuity of instruction during the student’s absence from the regular program.  Home & Hospital is not a credit recovery program.

Home & Hospital cannot be responsible for grading students two weeks prior to the semester ending.  In addition, Home & Hospital teachers cannot change prior grades or credits issued by the home school nor can Home & Hospital make-up tests, quizzes and missing assignments prior to the enrollment date into Home & Hospital.

A high school student can earn .25 credits per week per subject.  Students that enroll in Home & Hospital during the middle of a grading period will be issued quarter credit on their transcript (2.50 credits per subject) by blending the Home & Hospital grade with the withdrawal grade from the home school even if that grade is a “F”.

 

REQUIREMENTS

  1. Students referred to H&H must have a medical or psychological condition, verified by a licensed doctor’s signed authorization which requires the need for individual instruction from Home & Hospital for a minimum of five days from the date of the doctor’s authorization. (See Medical Professionals section)

  2. Parent must submit a signed release: Parent Authorization for Release of Information / Spanish

1. A referral form must be completed by a California licensed physician or psychiatrist (form located in the Medical Professionals section).

2. Parents must complete the parent release form and sign the bottom of the page.

These two completed forms should be submitted together to the student’s home school.  Meanwhile, a copy of the signed Release of Authorization and Referral Form can be emailed to homehosp@mdusd.org. The Home & Hospital Administrator will then contact you to discuss the logistics for your child's placement in Home & Hospital.

General education students in grades K-12 may be eligible for service from Home & Hospital if: they have a medically verified, non-contagious physical condition(s) or a temporary physical disability that cannot be accommodated at their home school.

Students that are school aged: grades 6-12, who reside in the hospital, will be provided academic services by the Mt. Diablo Unified School District (MDUSD). Home and Hospital services are five (5) hours per week. Sunol Hills staff will determine if a student is able to attend the school program.

Instruction is provided by credentialed teachers employed by the MDUSD. The H&H teacher will provide curriculum and materials. Secondary curriculum is provided in the areas of English Language Arts and Social Studies. H&H teachers are supervised by the Home & Hospital Administrator.

The MDUSD will inform the Home School District that the students are to receive academic services while in MDUSD attendance area.

High School students will receive .25 credits per subject per week, with successful engagement.

Grades are given for students with a minimum attendance of 2 weeks.

To enroll, parents must complete the release form below.  A psychiatric referral must also be completed and sent to homehosp@mdusd.org.
